These decide the deal on a DSCR loan. Estimates are fine — leave anything blank you don't know.

Actual rent if it's rented, expected rent if not. Blank is fine — market rent gets estimated for you.
Your best estimate is fine — no credit check happens now.
Per month — the yearly bill divided by 12 is fine.
